Introduction Aortic valve stenosis is one the most prevalent valvular disease with significant clinical burden. Its treatment has been revolutionized by transcatether aortic replacement (TAVR). Left ventricular function key element preprocedural patient assessment, however, conventional echocardiographic parameters are substantially influenced increased afterload associated disease. Myocardial work index a novel parameter that quantifies myocardial deformation in context concomitant left pressure, therefore, it may be reliable measure contractility even afterload. Purpose Our aim was to evaluate TAVR patients using and index, examine their association characteristics patients. Methods 84 candidates were enrolled (age 79±6 years; 48% female). Conventional advanced measurements performed prior procedure at 12 months as part follow-up visit. We measured ejection fraction (EF), also global longitudinal strain (GLS) speckle-tracking echocardiography, then volume curves estimated from systolic blood pressure mean transaortic gradient, we quantified (GWI). Detailed medical history, symptomatic status marker congestion, NT-proBNP levels obtained. Results EF did not change significantly (53±13 vs. 55±10 %; p = 0.172), while absolute value GLS (−13.5±4.5 −15.3±3.6 p<0.001). GWI decreased (1914±787 1666±594 mmHg%; 0.001) compared values (Figure 1). reduced cohort (1057 [586–3037] 627 pg/mL [376–1198]; Using multivariate regression analysis, examining relevant parameters, presence atrial fibrillation (β=0.31; p<0.01), (β=-0.32; p<0.01) proved independent predictors postprocedural (R²=0.30; Conclusions In our study population changed after procedure. The during follow-up. Accordingly, help improvement selection estimation prognosis this complex population.

Rise and fall of NT-proBNP in aortic valve intervention

Objectives To describe the dynamics of N-terminal proB-type natriuretic peptide (NT-proBNP) from preoperative evaluation to 6-month follow-up in patients undergoing aortic valve intervention, and to evaluate NT-proBNP with regard to 1-year mortality. Methods At preoperative evaluation, we prospectively included 462 patients accepted for aortic valve intervention. The median time to surgical aor...

Early Postoperative Anticoagulation by Enoxaparin after Mechanical Aortic Valve Replacement

Background: The use of low molecular weight heparin for early anticoagulation after mechanical aortic valve replacement is still a matter of debate even more that the early postoperative phase is associated with maximum of thrombo-embolic and bleeding risks. The objective of this study is to verify the efficacy and the safety of low molecular weight heparin for the early anticoagulation after m...

Transcatheter Aortic Valve Replacement: A Review Article

Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...

Left ventricular myocardial structure in aortic valve disease before, intermediate, and late after aortic valve replacement.

Left ventricular biplane cineangiography, micromanometry, and endomyocardial biopsies were performed in 27 patients with aortic stenosis (AS) and in 17 patients with aortic insufficiency (AI). Twenty-three patients with AS and 15 with AI were restudied at an intermediate time (18 months after successful valve replacement), and nine patients with AS and six with AI were restudied late (70 and 62...

Aortic regurgitation after transcatheter aortic valve replacement.

Paravalvular aortic regurgitation (AR) negatively affects prognosis following transcatheter aortic valve replacement (TAVR). As transcatheter heart valves (THV) are anchored using a certain degree of oversizing at the level of the aortic annulus, incomplete stent frame expansion because of heavily annular calcifications, suboptimal placement of the prosthesis, and/or annulus-prosthesis size-mis...
